Most of the information we need comes directly from you during the application process. We may contact you by phone, email, or mail to verify or update the details provided.

By providing your phone number, you consent to being contacted via calls, text messages, or automated communications, as allowed by law and for business purposes, including servicing your policy.  No mobile phone information or opt-in data will be shared with third parties/affiliates for marketing or promotional purposes.

In addition, we may obtain information from:

  • Personal Information: Details such as your name, address, Social Security number, and date of birth, obtained through applications, forms, phone calls, or emails.

  • Payment Information: Data required to process payments. Please note, we do not retain banking or credit card information on file.

  • Transaction Information: Records of your interactions with us, including claims, policy coverage, and payment history.

  • Your past insurers or insurance support organizations regarding previous claims.

  • Consumer reporting agencies, which may provide credit reports and other consumer data for underwriting or renewal purposes, as permitted by law.

  • State Motor Vehicle Departments for driving history (e.g., accidents, convictions).

  • Independent inspection companies, particularly for home, building, or boat insurance, to verify property conditions and value estimates.

We may also calculate an Insurance Score based on your credit history as part of our underwriting process. If there are errors in your credit information, we will reevaluate upon receiving corrected data.

Your insurance file may include policy details, payment history, and claim records. In some cases, we may need health-related information, such as for assessing driving ability. If required, we will ask you to authorize the release of medical information from your healthcare provider.

How We Protect and Use Your Information

We maintain your information in secure files protected by physical, electronic, and procedural safeguards.

We do not sell or share your personal information for marketing purposes. We only share information as permitted or required by law and for legitimate business purposes, such as:

  • Servicing your policy or settling claims.

  • Providing information to agents, brokers, or affiliates within our network.

  • Supporting scientific or actuarial studies (on an anonymous basis).

  • Preventing or addressing fraud, including sharing with insurance support organizations or other insurers.

 

We may also disclose your information under specific circumstances:

  • To comply with federal, state, or local laws or legal processes (e.g., subpoenas).

  • To state insurance departments or government agencies, as required.

Your Right to Access and Correct Your Information

You have the right to know what information we have about you and how it has been used. To request this information, please write to us. We will respond within 30 business days of receiving your request.

If we have shared your information with others, we will inform you of the recipient(s). If consumer reports were obtained, we will provide the name and address of the reporting agency.

You may review and copy your file, except for certain claim or legal documents. If you identify errors, we will correct them or allow you to submit a written statement to include in your file if we disagree. We will forward any corrections or statements to those who received the original information.

Text Messaging Communication Policy

The Michael P. Lavelle Agency Inc.  (an independent insurance brokerage and the agency’s employees) we use SMS (Short Messaging Service) and MMS (Multimedia Messaging Service). These text messaging services are used to provide various types of information to The Michael P. Lavelle Agency Inc. customers and individuals interested in our insurance products and services.

Recipients of these messages may receive recurring text messages at varying frequency from our Office’s phone number. Consent to receive text messages is not a condition of service from The Michael P. Lavelle Agency Inc., and message and data rates may apply.

You can stop receiving text messages from the office at any time. Just text "STOP" to the Agent Office’s phone number. After you send the text message you will be sent a confirmation message that validates your wish to unsubscribe.

If you want to receive text messages again, please contact the office phone number 516-747-6262 any someone in the office will send you an enrollment text message. Please respond “YES” to reenroll in text messages.

​

For further assistance, you can text "HELP" to the Agent Office’s phone. After you send the "HELP" message, we will respond with instructions on to call the office at 516-747-6262.

Message and data rates may apply for any messages sent to you from us and to us from you. If you have any questions about your text plan or data plan, it is best to contact your wireless provider.
